ADMISSION
PROCEDURE

Establish a date and time to meet with the Administrator to tour Paradise Manor and obtain all necessary information.The Administrator and Director of Nursing must complete a screening form to insure that Paradise Manor is able to meet the applicants needs.
The applicant must obtain a physical examination and a medical form (MA-51 is available from the office of Paradise Manor) certifying that the applicant is suitable for personal care.
Following approval by the Administrator, applicant and designated party will set a date for the applicant to enter Paradise Manor as a resident and to have all necessary paperwork explained and signed. Give list of what is required.
